The purpose of this notice is to explain to you how your protected health information (PHI) may be used and disclosed for the purpose of treatment, payment, and healthcare operations. It also provides you with information about your rights regarding your PHI, how you may access your PHI, and ask to have restrictions placed on the information about you that may be released without your authorization to another person or organization under the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
